2) Your takedown notice must include

  1. Your physical or electronic signature.
  2. Identification of the copyrighted work claimed to have been infringed.
  3. Identification of the material claimed to be infringing and information reasonably sufficient to permit us to locate the material (URL).
  4. Your contact information.
  5. A statement that you have a good‑faith belief the use is not authorized.
  6. A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the owner.

3) Counter‑notice

  1. Your physical or electronic signature.
  2. Identification of the material removed and its prior location.
  3. A statement under penalty of perjury that you have a good‑faith belief the material was removed due to mistake or misidentification.
  4. Your name, address, phone, and a statement consenting to jurisdiction of the appropriate courts and accepting service at the provided address.

Upon receipt of a valid counter‑notice, we may restore the material within the statutory period unless the complainant files an action seeking a court order.
