2) Your takedown notice must include
- Your physical or electronic signature.
- Identification of the copyrighted work claimed to have been infringed.
- Identification of the material claimed to be infringing and information reasonably sufficient to permit us to locate the material (URL).
- Your contact information.
- A statement that you have a good‑faith belief the use is not authorized.
- A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the owner.
3) Counter‑notice
- Your physical or electronic signature.
- Identification of the material removed and its prior location.
- A statement under penalty of perjury that you have a good‑faith belief the material was removed due to mistake or misidentification.
- Your name, address, phone, and a statement consenting to jurisdiction of the appropriate courts and accepting service at the provided address.
Upon receipt of a valid counter‑notice, we may restore the material within the statutory period unless the complainant files an action seeking a court order.
